PREDICTIVE VALUE OF CLINICAL SIGN OF HIP HYPER ABDUCTION FOR DEVELOPMENTAL HIP DISODER (RPK)

Abstract

Introduction: The aim of the paper is to evaluate the relationship between clinical sign of the hip hyper abduction and the developmental hip disorder (RPK). Materials and Methods: The study was conducted on 450 newborns at the Neonatal Department of Clinic for Gynecology and Obstetrics and Clinic for Orthopedics and Traumatology of the University Clinical Center in Tuzla, in the period from 30 .08. 2011. do 30.04.2012. On the first day, clinical and ultrasound examination of the hips was performed on all newborns born on the day of examination. Diagnosis of the developmental hip dysplasia was established by clinical and ultrasound examination of hips by Graf method. Clinical examination also showed the extent of the hip movement in the newborn. Results and discussion: The clinical sign of hip hyper abduction in the infant has a significant predictive value for the developmental hip disorder. There were 37 (8.2%) newborns with clinical signs of excessive abduction of the hip, of which 13 newborns had developmental hip disorder while 24 newborns did not have this disorder. The total sample was 89 newborns with one form of RPK, 13 (12.7%) had this clinical sign, while 324 newborns without RPK were 24 (6.9%) with this clinical sign. The excessive abduction of the hips had a positive predictive value (PPV) of 35.1% and a negative predictive value (NPV) of 78.5% for RPK. Conclusion: Hip hyper abduction in Newborns is an important and useful clinical sign for RPK. Doctors who make the first review of the baby after birth should be alert to this clinical sign. Newborns with this clinical sign should have an ultrasound examination of the hips in the first days of life.
